49ers Lose Sam Okuayinonu to Ankle Injury as Training Camp Casualty List Hits 20

The San Francisco 49ers can’t catch a break this summer. San Francisco’s 49ers injury report got longer on Wednesday when defensive lineman Sam Okuayinonu limped off the practice field with an apparent ankle injury, the latest blow to a roster that’s already stretched thin heading into the 2026 season.

The Athletic’s Matt Barrows was on hand for the injury. He posted on X: “The 49ers finished practice today without 20 players from their 90-man roster. Sam Okuayinonu went off the field with an apparent ankle issue, depleting an already thin position for SF.” —

Okuayinonu wasn’t the only one to go down. Offensive lineman Nick Zakelj also exited Wednesday’s session with an elbow injury, according to 49ers Webzone. Beat writer Noah Furtado put a number on the damage, posting on X that 18 of the 49ers’ 90 active roster players sat out that day’s practice — not counting five more on injured reserve, for 23 banged-up bodies total.

It’s a rough continuation of a pattern that’s dogged Kyle Shanahan’s tenure in San Francisco. The 49ers have repeatedly seen deep playoff runs undercut by attrition, and this camp has offered little reprieve. Wide receiver Ricky Pearsall is already out for the season, needing season-ending surgery, and free-agent addition Mike Evans has been sidelined as well.

What Okuayinonu Means to San Francisco’s Defensive Line

Okuayinonu isn’t a marquee name, but he’s meaningful depth on a defense that badly needs it. He started 12 games for San Francisco last season, racking up three sacks and 39 tackles while helping fill the void left by injuries elsewhere on the line. That included a run under then-defensive coordinator Robert Saleh, who left after one season in San Francisco to become head coach of the Tennessee Titans. Raheem Morris now runs the 49ers’ defense, and he’s inheriting a group that’s again shorthanded up front before Week 1.

Despite the growing list, the 49ers’ staff isn’t sounding alarms. Assistant head coach Chris Foerster — who has taken on expanded responsibilities while Shanahan works his way back from a July car accident — indicated the team’s plans for next week’s joint practices with the Tennessee Titans haven’t changed, according to 49ers Webzone.

Shanahan’s Own Recovery Adds to the Uncertainty

San Francisco’s injury concerns this camp extend to the sideline. Shanahan suffered a serious concussion, a broken nose, three broken ribs and a broken hand in a July 14 car accident near his Northern California home, according to ESPN. He’s taken responsibility for the crash, saying he reached for a dropped phone and crossed the center line. General manager John Lynch confirmed Shanahan is still dealing with lingering concussion symptoms and is following a return protocol similar to what players go through, with Foerster and the coordinators handling day-to-day duties in the meantime.

Where Nick Bosa — and Joey Bosa Speculation — Stand

One bright spot for San Francisco’s defense: Nick Bosa looks ready to go. The star pass rusher, who tore his ACL last September and needed season-ending surgery, was cleared at the start of training camp. Teammates have noticed. “I couldn’t even tell he had an injury or he came back from anything,” left tackle Trent Williams said, via 49ers Webzone. Bosa is expected to be back for San Francisco’s Week 1 opener against the Los Angeles Rams in Melbourne, Australia.

Bosa’s return has fueled recurring chatter about his brother, veteran pass rusher Joey Bosa, joining San Francisco as a free agent. But that speculation has cooled. Lynch told reporters earlier this offseason he wasn’t confident the 49ers could afford a deal, and Nick Bosa himself said in May his brother was more focused on golf than football, per Niners Nation. With San Francisco already tight against its 2027 cap projections, a reunion remains unlikely for now — Wednesday’s injury news notwithstanding.

San Francisco is scheduled to hold joint practices with the Titans next week, and the medical staff will spend the coming days evaluating Okuayinonu’s ankle. The 49ers open the regular season on the road against the Rams in Australia, with training camp health now a genuine storyline instead of an afterthought.
